SkillWise collects some of your personal information to support you in the best way possible.
This helps us give you the right disability and education support.

What information we collect.

  • Your name and contact details

  • Information about your family, whānau and support people

  • Your NHI, SWN, or IRD number

  • Details about your funding or benefits

  • Your health and disability information

  • Photos or videos (only if you agree)

How we collect your information.
We like to collect your information:

  • Directly from you

  • Or from someone you trust (like your family, whānau or support worker)

Sometimes, we may ask other professionals (like doctors) for health information to help support you.

You can tell us if you're worried about this.

What we use your information for.
We use your information to:

  • Support you properly

  • Stay in contact with you or your key people

  • Run our services

  • Meet legal or government rules

We may also:

  • Share information with government agencies (like the Ministry of Social Development or Ministry of Health)

  • Share info with other professionals who support you

  • Use information for staff training and to improve our services (your name will not be used)

We only use your information for these reasons — or if the law says we must.

How we keep your information safe.

  • Your information is stored safely in our computer systems

  • Only SkillWise staff and volunteers can see it

  • We do not share your personal information with others unless you agree or the law requires it

We keep your records for 10 years after you leave SkillWise.

Your rights

You can:

  • Ask to see your information

  • Ask us to fix it if something is wrong

  • Ask us questions or make a complaint if you're worried about your privacy
