Photos or videos (only if you agree)

We like to collect your information:
Directly from you
Or from someone you trust (like your family, whānau or support worker)
Sometimes, we may ask other professionals (like doctors) for health information to help support you.
You can tell us if you're worried about this.
What we use your information for.
We use your information to:
Support you properly
Stay in contact with you or your key people
Run our services
Meet legal or government rules
We may also:
Share information with government agencies (like the Ministry of Social Development or Ministry of Health)
Share info with other professionals who support you
Use information for staff training and to improve our services (your name will not be used)
We only use your information for these reasons — or if the law says we must.
How we keep your information safe.
Your information is stored safely in our computer systems
Only SkillWise staff and volunteers can see it
We do not share your personal information with others unless you agree or the law requires it
We keep your records for 10 years after you leave SkillWise.
